Summary

Description A U.S. Navy Douglas A-4E Skyhawk (BuNo 152048, "Lady Jessie", pilot LCdr. R.C. Perry) from Attack Squadron VA-164 "Ghost Riders" in flight. VA-164 was assigned to Carrier Air Wing 16 (CVW-16) aboard the aircraft carrier USS Oriskany (CVA-34) for a deployment to Vietnam from 26 May to 16 November 1966.
LCdr. Perry was shot down and killed by a SAM on 31 October 1967 while flying A-4E 151991 (side number AH-407). The A-4E 152048 was shot down by ground fire over North Vietnam on 18 October 1967 while in service with VA-164. The pilot LCdr. Barr was also killed.
