Summary/Objective

Concerto is seeking a Social Work Team Lead to join its team! The Lead Social Worker is proficient in all social work roles and responsibilities with an emphasis on team leadership, including management of direct reports. Primary responsibilities include new hire precepting, ongoing education, auditing of social work tasks to ensure compliance, and working closely with the Manager and Director of Social Services on process improvements and initiatives. Other areas of focus include building and maintaining a healthy team culture and cross functional relationship development. The Lead Social Worker is also committed to personal growth and development opportunities.

Essential Functions

Team Management

  • PDR
  • Assists with timecard approval for hourly staff.
  • Collaborates with Manager of Social Services on workforce planning, including interviewing prospective team members.
  • Initiates corrective action process when necessary, collaborating with Manager/Director and Human Resources to optimize employee performance.

Team Lead Responsibilities

  • Conducts regular check-in’s with all Social Workers in their market
  • Facilitates monthly meetings
  • Manages compliance, including Resnesan audit
  • Oversees on site onboarding of new hires, including precepting, as needed
  • Ongoing team education
  • Plans teambuilding activities
  • Facilitates monthly QAPI in all assigned markets

Patient Care

  • Manages individual case load in assigned markets

Clinical Services Partnership Projects

  • Metric reporting

New Clinic Openings

  • Initiates and completes admissions paperwork reporting status to Manager, Director, and Intake team
  • Conducts in-services

Insurance & 2728’s

  • Functions as subject matter expert for the Insurance Navigator Program and 2728 support within assigned markets to educate, track, and roll up to Manger and Director

Clinical Staff Education/In-Services

  • Provides training to clinic staff, including but not limited to:
    • Sensitivity training
    • Renasan
    • Other topics as requested by Regional Managers, market VP’s, or Director of Social Services.
Qualifications

  • Master’s degree in social work.
  • Healthcare or dialysis related background required
  • Must be licensed as an LSW or LCSW.
  • Unencumbered driver’s license.
  • Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment.

Physical Demands Of The Job

  • Must be able lift at least 20 lbs.
  • Requires mental, visual, and auditory attention to patients, customers and staff.
  • Work environment is characteristic of a health care facility with air temperature control and moderate noise levels.
  • May be exposed to infectious and contagious disease/materials.
  • Requires regular out-of-state travel.
