Case Manager ECM

Job Purpose

The ECM Case Manager is responsible for outreach, engagement, and providing support and linkage to wellness/recovery resources, both in person and virtually to assist clients in reaching the optimal level of wellness and functional capability. The ECM Case manager works with highest risk Medi-Cal members with the most complex medical and social needs, connecting with all their providers to coordinate areas of care and support.

Duties And Responsibilities

  • Engages and enrolls community members eligible for ECM per care plan protocols
  • Assesses client need areas
  • Develops an individualized care plan with each client that is updated as needed
  • Coordinates care for clients with a multidisciplinary clinical team and outside agencies.
  • Facilitates referral linkages to community resources and performs case management services.
  • Assist clients with the development of social and community support systems.
  • Develops effective working relationships with agencies, organizations and providers to advocate for clients.
  • Documents and maintains clinical records per appropriate standards and regulations.
  • Conducts outreach in diverse community settings.
  • Participate in clinical supervision, consultations, and trainings.
  • Provide translation services as needed.
  • Complete other duties and related projects as assigned.

Qualifications

  • Bachelor’s degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above.
  • One year or more of case management or related experience.
  • Familiarity with community-based resources.
  • Excellent problem-solving and organizational skills.
  • Understanding of cultural issues, community resources and family dynamics.
  • Ability to communicate well with people of diverse cultural, professional, and experiential background.
  • Ability to establish and maintain effective work relationships as part of a multidisciplinary team.
  • Ability to maintain a valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.
  • Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) preferred
  • Trauma informed training preferred

Working conditions

This job requires you to be in the community, in client’s homes, schools, social justice offices and other community partner sites as well as work in one or more of our office locations. Driving is essential to the function of this job.
