Asylee and Refugee Case Manager

Job Purpose

The Peer Partner provides guidance and support to assist clients in understanding and navigating public support systems. Responsibilities include conducting outreach and training within the community, acting as a liaison with community partners, and representing the CST program. As part of a collaborative team, the Peer Partner will also serve as a liaison with other service providers for the benefit of clients.

Duties And Responsibilities

  • Facilitate referrals to community resources and perform case management services.
  • Assist clients in establishing and nurturing social and community support networks tailored to their needs.
  • Develop effective working relationships with agencies and organizations to advocate for clients’ rights and needs.
  • Complete timely and accurate documentation in adherence to Medi-Cal, Santa Clara County, and AACI standards.
  • Engage in proactive outreach initiatives within the community.
  • Participate in clinical supervision, consultations, and training.
  • Provide translation and cultural brokerage.
  • Accompany clients to appointments in the community as needed.
  • Complete other duties and related projects as assigned.

Qualifications

Education & Experience:

  • BA in Social Work or similar field or equivalent experience.
  • One year or more experience with case management.

Knowledge, Skills, & Abilities

  • Knowledge of community and county resources to facilitate referrals and coordination with appropriate agencies.
  • Proficient in using commonly used technology tools and devices including computers, smart phones, email, and internet.
  • Working knowledge of Microsoft Office
  • Ability to maintain a valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability.
  • Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese).

  • Ability to communicate well with people of diverse cultural professional and experiential background.
  • Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Working conditions

This job requires you to be in the community, in client’s homes, and other community partner sites. Work is often conducted in an office environment. Basic safety precautions and use of protective clothing or gear may be required. Working with clients who are seriously mentally ill, or high risk may increase exposure to secondary trauma. Driving is essential to the function of this job. The job requires travel up to 25% of the time.
