You’ll serve as Homecare social worker for patients and their families, providing support, therapeutic interventions, education, and patient advocacy tailored to the needs of the specific client population. You will be part of the Homecare interdisciplinary team providing excellent care to our clients and families. Here, you will experience all the reasons why you entered such a rewarding field.

This is a full- time position covering varied days, from 8:30am -4:30pm located at Lawrence Homecare of Westchester, a community-based Certified Home Health Agency. You will be required to go into patient homes and other facilities based on the patient’s service location.

Preferred Criteria

• State and local community services
• Homecare experience preferred
  
  

Required Criteria

• Master's Degree in Social Work (MSW)
• Licensed Masters Social Work (LMSW) by the state of NY within 1 year of hire
• Knowledge of Psychosocial Assessment
• Communication Skills

Salary Range

$82,500-$105,000/Annual
