Job Summary

Under the supervision of the Mental Health Services Senior Case Manager, the Mental Health Services Case Manager provides case management services for clients in the Community Mental Health Services program. The Mental Health Services Case Manager coordinates care with intra-agency staff, county workers, school personnel, referring agencies and the community at large. In addition, they serve a caseload of children and/or adolescents in the school, home, and community settings. In addition, the Mental Health Services Case Manager introduces and educates the community on the agency’s services through trainings, public speaking, and networking with a goal of gaining new referrals for the Mental Health Services Program. The Mental Health Services Case Manager must also submit all required documentation in accordance with Department of Mental Health regulations.

Essential Functions

  • Consistently serve a caseload of children and/or adolescents with Individual Rehabilitation, Targeted Case Management, and Collateral services in the home, school, and community setting
  • Represent Aviva within the community through providing Community Outreach Services (COS) to help engage individuals not yet receiving mental health services.
  • Introduce, explain, and educate schools, other mental health agencies, health clinics, resources fairs, parent groups on the benefits of Aviva’s Mental Health Services and Evidence-Based Practices.
  • Participate in community outreach events
  • Complete referral forms for prospective clients
  • Complete and submit all documentation and Case Records Reviews in accordance with agency and Department of Mental Health regulations (including, but not limited to progress notes and Client Treatment Plan goals
  • Consistently meet agency billing productivity requirement
  • Meet weekly with supervisor and consult as necessary when clinically indicated (e.g., potential Suspected Child Abuse Reports, other crisis situations)
  • Maintain effective working relationships with and coordinate care with intra-agency staff, county workers, school personnel, referring agencies, and the community at large
  • Sensitive and responsive to the cultural differences among the agency’s client population
  • Actively participates in quality improvement projects

The Mental Health Services Case Manager will take on special projects or additional duties as assigned by the Mental Health Services Senior Case Manager.

Competency Requirements

  • Ability to speak, read, write, and understand English
  • Ability to work effectively as a member of the treatment team
  • Ability to promptly and appropriately respond to a crisis situation
  • Must have excellent oral and written communication skills
  • Must demonstrate attention to detail
  • Must have excellent customer service skills and phone manner
  • Bilingual (English/Spanish) required

Education And Experience

  • Bachelor’s degree in related field required
  • Previous experience working in a social services agency in a similar capacity preferred

Certifications/Licenses

  • Valid CA driver’s license

Additional Requirements

  • Tuberculosis clearance
  • Department of Justice/Fingerprint clearance
  • Satisfactory completion of health screening
  • Proof of legal eligibility to work in the United States
  • Satisfactory driving record

Physical Demands

While performing the duties of a Mental Health Services Case Manager, the employee is frequently required to sit, stand, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to walk, bend, drive, and lift objects heavier than ten pounds. The employee is rarely required to run.

Work Environment

The work environment is primarily indoors. The noise level varies from quiet to moderate.
