Job Description

Duties shall include:

  • The SCARNG has identified this position as a key personnel position.
  • The industry standard for the educational requirements for an Administrative Care Coordinator is a High School Diploma and with a minimum of three years’ experience related to Military Health care.
  • Contractor shall maintain Soldier medical/ dental records in accordance with Army Regulation (AR)40-66.
  • The contractor shall utilize the File-On-Q program to sign out and in all medical records to ensure 100% accountability of SM records.
  • Contractor shall mark and safeguard files, output products, and storage media per classification level and disseminate them only to individuals authorized to receive them IAW HIPAA.

  • Contractor shall create medical records for new accessions, prepare and process existing medical records for individuals scheduled for Mobilizations, Military Training, Schools, Transfer, Discharge, Retirement, and all IMRX/SRP events.
  • Contractor shall file provider notes, imaging, and lab documentation in individual medical/dental records in both the hard copy file and in the electronics Health Readiness Record module in MEDCHART.

  • Contractor shall ensure all administrative documentation in MEDCHART (eCase and DENCLASS) and MODS (MEDPROS, MHA/PHA) is completed weekly.
  • Contractor shall follow up and monitor medical readiness databases daily to ensure all scheduled services are completed/ updated and current medical status is reflected.
  • Contractor shall assist Case Management staff to ensure the SM MEDPROS data is accurate and up to date before each IMRX AND SRP to assist with early identification and follow up of medical conditions that may require a profile update or initiation of a board process.
  • Contractor shall contact mobilization stations and coordinate the preparation, packaging and shipment of Service Members’ Adult Preventive and Chronic Care Flowsheet DD Form 2766 (Enclosure 1) in accordance with AR 40-66 for shipment to the mobilization platforms as prescribed.
  • Contractor shall perform quality assurance checks on new in-coming medical/dental records and tracks all discrepancies to closure monthly.
  • These checks need to be completed on-time and discrepancies tracked to meet their suspense.
  • Contractor shall participate in weekly case reviews to ensure that every SM with an open case is being tracked, managed, and communicated with at a minimum of every 45 days through MEDCHART and MEDPROS.
  • Contractor shall alert the case manager(s) of automated referrals indicated n DENCLASS / MEDCHART / eCase.
  • Contractor shall provide medical/dental documentation, and profile concerns to the case manager/dental case manager for review and submit to the appropriate military provider.
  • Contractor shall assist with and attend all IMRX AND SRP events.
  • The contractor will be notified by the COR at least 30 days in advance of the event.
  • Contractor shall follow the regulations and policies applicable to IMR and deployment, to include AR40-501, MOD 14 (Amplification of the minimal standards of fitness for deployment to the USCENTCOM AOR).
  • Contractor shall ensure all medical/ dental documentation received from SMs and/ or medical professionals is received and uploaded in appropriate electronic record, as well as the SM’s hard copy record.
  • This includes filing of all medical/dental documents received for case management within the STR and stamping all medical/dental documents for verification after uploading.
