The Clinical Supervisor is responsible for providing leadership, supervision, and support to clinical Interdisciplinary Team (IDT) departments.

Specific Responsibilities

  • Leadership
    • The Clinical Supervisor is a key member of the On Lok PACE at IOA Leadership Team.
    • Works collaboratively with supervised staff and peer leaders to ensure smooth operations and high-quality care that is fully integrated with the interdisciplinary team (IDT). Develops and refines workflows
    • Builds strong teams to promote engagement and foster quality of care.
    • Ensures appropriate systems are in place for service delivery.
    • Creates a work environment conducive to safe work practices, effective communication, staff collaboration and positive morale.
    • Encourages an environment where diversity is valued and encouraged.
    • Provides evening and/or weekend administrative phone coverage on a rotational basis.
  • Supervisory Responsibilities:
    • Supervises staff in the social work, rehab, and dietary departments. Completes hiring, on-boarding, competency training and testing, performance reviews, and disciplinary actions.
    • Assures adequate coverage and scheduling management.
    • Supports professional development and clinical training.
    • Ensures that staff are effectively performing and participating on the IDT for participant care and coordination, by providing coaching and training, when indicated.
    • Participants in the review and development of policies and procedures that address the needs of the program, particularly in the areas supervised.
  • Quality and Compliance
    • Ensures compliance with regulatory requirements, adherence to policies and procedures, and participates in continuous process improvement projects and risk management efforts.
    • Working with the Leadership Team, develops goals and implements corrective action plan to manage utilization of services ensuring participant needs are met in an efficient manner.
    • Leads and/or participates in improvement projects related to employee and participant satisfaction surveys.
  • Clinical Care and Care Planning
    • Oversees the provision of high-quality rehab, nutrition, and social work services to PACE participants.
    • Provides direction ensuring continuity and consistency of participant care planning across the teams and between disciplines.
    • Implements a best practice approach assuring staff are trained to provide services effectively and efficiently.
    • Facilitates IDT meetings, ensuring healthy team dynamics and optimal care coordination.
    • Other duties as assigned.

    Required Qualifications

    • At least 3 years clinical experience as a master’s level social worker, PT, OT, SLP, or RD.
    • Current BLS certification (CPR) or willingness to obtain within 6-months of hire.
    • Ability to work within the interdisciplinary team with initiative, imagination, resourcefulness, and flexibility.
    • Knowledge of the community served.
    • Interest and willingness to work in a multilingual, multicultural environment.
    • One-year experience working with the elderly or frail required.
    • Ability to comprehend complex written materials and verbal instructions.
    • Computer literacy.

    Desired Qualifications

    • Possesses a passion for working with a geriatric population.
    • Some leadership experience, or a desire to gain leadership experience in a healthcare setting.
    • Experience with diverse cultures and communities.
    • Knowledge of best practices for clinical and behavioral interventions.
    • Demonstrated ability of being flexible in times of change.
    • Demonstrated excellence in working in a team environment.
    • EPIC electronic medical records experience preferred.

    Physical Requirements

    • Must be able to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
    • Ability to reach, bend and walk
    • Five finger manipulation

    Compensation

    • Range: $100,958.90 - 109,681.74/Annual

    This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.

    This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
