PROVAIL has an exciting opportunity for a Team Coordinator to join our team! In this role, you will Supervise day-to-day operations of assigned residences. Coordinate provision of person-centered services for each client in assigned residences.

What You’ll Do

  • Supervises assigned Direct Support Staff (both regular and on-call) for assigned houses, including orientation, coaching, training, motivation, performance evaluation and performance counseling. Facilitates a team-based environment in which staff cooperate to provide optimum support and instruction to clients. Ensures accurate and timely recordkeeping of time and attendance.
  • Schedules staffing to effectively support clients’ health, goals and choices within the limitations of contracted hours. Posts monthly schedules and meeting/training announcements well in advance and works with staff to accommodate properly submitted requests for time off. Promptly reports needs for additional staffing, and provides documentation to support the request.
  • Ensures that proper supports are provided and procedures followed to protect client health and safety, including providing an appropriate living environment. Conducts regular evacuation drills, water temperature testing, abuse/neglect checks and/or body checks, and submits reports in a timely manner. Responds to emergencies, attends annual and specialist healthcare appointments and ensures thorough follow-up, such as return visits or referrals, medication administration, instructions to staff, etc. Reports and document service refusals.
  • Supports client choice, goals and community inclusion. Coordinates planning with client, staff, family/guardians, case manager and other support providers. Monitors client goal progress and documentation. Ensures that all staff understand each client’s goals and how to support them.
  • Understands and implements current state regulations and policies, and agency policies and procedures. Ensures timely and complete reporting of client incidents, and provides appropriate follow-up to ensure client health and safety. Regularly reviews policies and procedures with staff during house meetings.
  • Ensures thorough, proper documentation of activities related to client health, goal progress, and staff assistance with client financial recordkeeping. Completes IISP reviews at least every six months or as goals change. Documents attendance at trainings and meetings.
  • Drives PROVAIL vehicles and assists clients in scheduling use of transportation resources.
  • Provides hands on direct care as needed.
  • Represents PROVAIL’s Diversity, Equity, and Inclusion practices by ensuring that all staff are treated fairly, with respect, and with consideration of their cultural, gender, linguistic, and other identities.

Additional Responsibilities

  • Monitors the appropriate use of PROVAIL supplies and equipment, including, but not limited to: computers, printers, faxes, etc.
  • Performs similar or incidental job duties as needed.

What You’ll Bring

  • Associates Degree or equivalent work experience in social services.
  • Registered as a Nursing Assistant (or Certified Nursing Assistant).
  • Minimum of two years experience working with people with developmental disabilities.
  • At least two years experience in any supervisory capacity.
  • Must be able to safely perform client transfers, lift, push, pull, and carry a minimum of 50 lbs.
  • Ability to communicate effectively with clients, parents or guardians, case managers, PROVAIL staff, and management.
  • Must have current and valid Washington’s driver’s license and good driving record, own vehicle and maintain insurance acceptable to the agency’s insurance carrier.
