Crestline is seeking a Social Service/Admissions Director. The position must function as a team member, team leader, and supervisor to ensure that work is accomplished, and quality care is delivered, supporting team members and taking direction from the Executive Director.JOB DUTIES & RESPONSIBILITIES:Develops and maintains a good working rapport with other direct caregivers to assure their participation and support of all programs/services to benefit the residents’ psychosocial wellbeing.Assist in identifying and correcting problem areas and determines the proper approach to any questions.Always maintains confidentiality of all resident/family information.Maintain proper documentation and perform general duties.Perform other related activities as assigned or requested.Assist in coordinating the referral process from the identification of the appropriate payer source to the analysis of patient informationCollaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members and social work staff members to ensure seamless transfer to the facilityCommunicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely mannerOther duties as assigned by supervisorQUALIFICATIONS AND EXPERIENCE REQUIREMENTSPrevious experience in admissions in a nursing facility.Previous long-term care experience.As a Crestline employee, you will enjoy competitive wages and PTO plans. Requirements/Qualifications

  • Previous long-term care experience.
  • Previous experience in admissions in a nursing facility.
  • Experience in Admissions or Social Work
