Many of our Primary Care practices have Behavioral Health specialists as members of the care team to ensure collaboration and communication with patients' primary care providers.

About The Job

Responds to al behavioral health requests for new patient appointments. Responsible for coordinating provider schedules and interfacing with multiple practices to ensure appropriate referrals, conducting initial patient intakes to assess immediacy or feasibility of care for FMP and/or external community resources.

What You’ll Do

• Completes initial phone screening and new intake forms with new referrals on a timely basis.
• Schedules new patients to behavioral health provider based on intake assessments.
• Communicates effectively with a variety of individuals representing diverse cultures and backgrounds and function calmly in high stress situations, requiring a high degree of sensitivity, tact and diplomacy.
  
  

Who You Are

• Associate’s Degree in Sociology, Psychology, or Human Services
• Bachelor of Science in Sociology, Psychology, or Human Services, preferred.
• Minimum requirement of 1 year of experience in healthcare setting required.
• Experience in mental health care setting preferred.
