Dependable Hospice & Palliative Care is seeking a compassionate and hardworking Hospice Registered Nurse to join our team. As a Hospice RN, you will provide specialized care and support to patients and their families during end-of-life journeys. You will collaborate with interdisciplinary teams to create and implement individualized care plans for patients in a comfortable and supportive environment. RESPONSIBILITIES:
Provide expert care and support to hospice patients and their families in consultation with interdisciplinary teams
Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
Administer medications and treatments as prescribed by physicians
Monitor and document patient progress, and make any necessary adjustments to care plans
Educate patients and families on how to cope with end-of-life issues and facilitate discussions about advance directives
Evaluate and manage symptoms such as pain, anxiety, nausea, and shortness of breath
Collaborate with social workers, chaplains, and other team members to create safe and effective care plans
Provide emotional support to patients and families as they navigate the end-of-life process
Maintain up-to-date knowledge of hospice care regulations and best practices
Participate in interdisciplinary team meetings and communicate effectively with team members
Complete documentation and billing in a timely and accurate manner
Participate in on-call rotation as necessary
JOB REQUIREMENTS:
Valid and current RN license
Minimum of 1-year nursing experience in hospice or palliative care preferred
Knowledge of hospice care regulations and best practices
Strong assessment and critical thinking skills
Ability to work effectively in a team-based environment
Excellent communication and interpersonal skills
Ability to provide compassionate and supportive care to patients and families
Ability to complete documentation and billing in a timely and accurate manner
EDUCATION:
Associate's or Bachelor's degree in nursing from an accredited institution
Current RN license
