Summary

Job Description

Under the direction of the Program Manager and other designated supervisor, s/he will be responsible for:

Essential Functions

  • Conduct education and promotional outreach services.
  • Distribute program brochures.
  • Assist to screen potential clients for eligibility for services.
  • Conduct telephone intake of community referrals.
  • Assist in providing linkages to services for clients as outlined by the client care plan under the consultation of Psychiatric Social Workers (PSW) and/or Care Managers (CM).
  • Assist PSW and/or CM with follow-up phone calls and client monitoring.
  • Participate in Care Plan Team conference meetings.
  • Assist in maintenance of developing, listing, and updating the project’s resources and database.
  • Conduct data entry utilizing designated Management Information System and/or other Information Systems.
  • Answer telephone calls and if necessary provide appropriate referral.
  • Complete necessary activity documentation in client clinical records and complete other monthly program documentation as needed.

Other Duties:

  • Participate in on-going staff trainings.
  • Work on other appropriate tasks as assigned by the Program Manager and/or designated supervisor.

Secondary Functions

  • Assist in the collection of appropriate client monthly reports.

Minimum Qualifications - Knowledge, Skills And Abilities Required

  • High School Degree with minimum of 2 years of community work experience and/or completion of 2 years college course work in health, social work, psychology, and related field.
  • Knowledge of community resources.
  • Sound personal qualifications (i.e. maturity, stability, organized, and positive attitude).
  • Ability to work effectively with people, particularly with culturally/ethnically diverse communities.
  • Ability to utilize computer office software (i.e. Word, Excel, PowerPoint, Outlook, etc.).
  • Able to work nontraditional hours and weekends.
  • Ability to communicate well in a team setting.
  • Possess a valid California Drivers License
  • A personal vehicle with appropriate automobile insurance.
  • Must be eligible to work in the U.S.
  • Fully vaccinated against COVID-19

Non-Essential Qualifications

  • Bilingual skills, especially API languages and Spanish, a definite PLUS but not required

Environmental Conditions (Working Conditions)

  • Must be able to work in the community (field) based settings. Requires driving in all weather conditions as well as challenging neighborhoods throughout Los Angeles County. Conditions may also involve working with challenging clients in the home setting. Home settings may involve unpleasant hygiene, odor, and some exposure to second-hand smoke.

Physical Requirements

  • Able to spend time sitting, driving, and maneuvering up and down stairways.
  • Some lifting of office equipment, supplies up to 30 lbs.
  • Able to listen and communicate verbally

Mental Requirements

  • Requires ability to adjust and adapt to changing priorities/work schedules.
  • Possess stable and sound mental health.
