Reports To: Program Manager

Summary: Responsible for conducting community education and outreach activities in order to increase Medicare and Health Insurance Counseling and Advocacy Program (HICAP) program awareness throughout the San Francisco County.

Essential Functions

  • Promotes greater awareness of Medicare benefits and HICAP services to Medicare beneficiaries and their families throughout the San Francisco County; develops and implements an outreach work plan in collaboration with program staff.
  • Distributes outreach materials; identifies need for additional outreach materials (e.g. brochures, informational pamphlets, press releases, announcements, flyers, posters and signs) and assists with the development of materials in appropriate languages.
  • Participates in outreach roundtable discussions, relevant workgroups, community partnership committees and training sessions as assigned by the Program Manager; attends various local community meetings and disseminates the information to staff members.
  • Develops and maintains positive working relationships with other agencies and healthcare providers in the community which coordinate the Area Agency on Aging on Affordable Care Act implementation.
  • Documents services/activities and updates records in the SHARP/Peerplace System and Google Calendar according to program protocol requirements and the California Department of Aging (CDA) website; submits monthly service reports as assigned by the Program Manager.
  • Recruits and trains Community Educators from the pool of volunteer counselors who meet the criteria, coordinates public and media event schedules, and provides ongoing training when needed.
  • Maintains and updates the calendar and information on the HICAP website.
  • Provides HICAP counselors with technical support as needed.
  • Assists the Program Manager in ensuring program compliance with its contractual obligations and community relation performance meets required attainments.
  • Provides HICAP telephone or in-person counseling as needed.
  • Performs other duties as assigned.

Qualifications

  • Bachelor’s degree in social service or related field; or college education with at least three years of experience in social services, health care, or elderly and adults with disability community. Experience in special event development and implementation preferred.
  • Knowledge of Medicare, Medi-Cal, MediGap, managed care and long-term care insurance preferred.
  • Must complete HICAP counselor registration within 3 months of employment.
  • Excellent verbal and written communication and interpersonal skills; and solid experience in group presentations. Bilingual in Cantonese, Spanish, or any Southeast Asian languages preferred.
  • Ability to work independently and in a team environment.
  • Proficient in MS Office and desktop publishing.
  • Must be available to work weekends and evenings occasionally.
  • Must pass a strength test for physical fitness for the job: able to lift or carry up to 25 lbs, re-testing every two years.
  • Must have and maintain a valid CA driver license with a satisfactory driving record.
  • Must be sensitive to the needs of elderly from diverse minority cultures and LGBTQI communities.
