OVERVIEW: The Family Partner ensures families are equal partners in the planning, development and implementation of mental health services at the Sacramento Children’s Home. The Family Partner supports that agency policies and systems are family friendly, culturally competent and stigma-free at all levels of the agency. The Family Partner works on integrating advocacy services into the mental health programs. The Family Partner provides peer support to eligible caregivers and guidance in linking to and navigating various systems.

DUTIES:

  • Direct Advocacy Services

  • Work directly with assigned caregivers to provide one on one support and peer counseling.

  • Participate in the development and implementation of family friendly and consumer-driven services within new and current programs.

  • Participate in treatment planning, case conferences, etc. as appropriate.

  • Provide support, coaching and assistance to caregivers with education rights during special education processes as needed.

  • Provide client and family with support towards stabilization of housing for the purpose of preventing or addressing homelessness.

  • Collaborate with team, clients/caregivers to develop housing plans.

  • Develop relationships with landlords.

  • Partner with clients/caregivers to address housing barriers and provide resources as needed.

  • Track monthly reporting of assistance provided, at-risk status and resolution.

  • Calculate rental assistance and provide information regarding SCH assistance policy.

  • Provide intensive case management to support self-sustaining housing.

  • Provide individual advocacy, education, and support to meet the needs of families, to include identifying systemic barriers to participating in services.

  • Provide the parent/caretakers’ perspective and feedback to the clinical staff in order to increase staff awareness of the needs of families and possible barriers to meeting the client’s goals.

  • Provide support and advocacy to families navigating various systems (i.e. school, housing, CPS, Alta Regional Center, etc.), as appropriate.

  • Provide information to parents/caregivers to help engage them in forming a positive relationship with treatment team members. Support parents/caregivers in self advocacy and in making informed decisions regarding the care of their children.

  • Document all visits with families/clients and services provided.

  • Design, implement and facilitate Caregiver Support group for Caregivers being served in SCH Mental Health programs.

LOCATION: 2750 Sutterville Rd., Sacramento, CA 95820

SCHEDULE: Monday – Friday, 8:30 am – 5:00 pm

SALARY: $21.39 – $24.07 per hour to start, PLUS up to $8,000 annual productivity bonus

Qualifications

Education and Experience:

A High school diploma or GED is required.

A BA degree in Social Work, Psychology, or a related field is preferred. Previous experience working in a mental health or social service setting is preferred.

Must have experience as a parent or guardian of a child with serious emotional disturbances who received services within the public systems including mental health, child welfare or juvenile justice.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
