Responsibilities:
The Physical Therapist directs physical therapy interventions aimed at preventing disability, alleviating pain, enhancing, restoring, and maintaining optimal muscle function within the client’s capabilities, following the prescribed care/treatment plan.
Supervise physical therapy assistants to ensure the proper implementation of the care plan.
Evaluate the client’s functional level through diagnostic and prognostic tests assessing muscle, nerve, joint, and functional abilities.
Assess the home environment for rehabilitation and recommend appropriate enhancements to enhance the client's independence.
Deliver physical therapy services at the client’s residence as prescribed by a physician.
Administer manual therapeutic exercises, guide and assist clients in non-manual exercises, and facilitate functional activities.
Provide traction, massage, and treatments involving the application of physical agents.
Evaluate, fit, and adjust prosthetic and orthotic devices, recommending modifications to the physician.
Contribute to the development, review, and update of the care plan.
Observe, record, and report any changes in the client’s condition and response to therapy, making suggestions for modifications to the care plan to the client’s physician and Case Manager.
Prepare clinical records, including initial evaluations (including OASIS), care plans, progress notes, and discharge summaries, submitting them for the client’s clinical record.
Participate in interdisciplinary team case conferences, contributing to planning and evaluating the overall treatment for the client, and confer with team members as necessary.
Advise and consult with the client/family and other agency personnel.
Submit orders and documentation (clinical and payroll) promptly and in accordance with branch policy.
Instruct the client/family on the use and care of physical therapy devices and the client’s comprehensive physical therapy program.
Instruct and supervise Physical Therapy Assistants in specific phases of physical therapy services they may provide.
Supervise the Physical Therapy Assistant minimally as required by applicable state law.
Complete an on-site functional assessment, review and revise the care plan, and assess the utilization of community resources as required by the client.
Be accessible to the Physical Therapy Assistant when providing treatment to a client.
Fulfill other assignments and duties as requested and assigned to achieve the goals and objectives of the Company.
Qualifications:

Doctor of Physical Therapy (DPT) degree
One (1) year of experience as a physical therapist in acute care or home care settings, or as mandated by law
Current, in-good-standing PT licensure in the state(s) of practice
Valid CPR certification
Possession of a valid driver’s license and automobile insurance
Strong dedication to clinical excellence, quality, and integrity

Excellent interpersonal communication and patient/family engagement skills

Proficiency in federal, state, and governing agency regulations

Ability to use professional judgment in delegating selected nursing tasks
Strong capability to be the public face of the organization and serve as an advocate for Home Healthcare, Home Hospice, and other service lines
Salary:
$90,000 - $100,000 a year plus full benefits.
