The member will enroll and recruit Advocate patients to participate in the Healthy Living Program Food Farmacy. The member will coordinate food distribution and assess the program’s impact through several measures including qualitative and quantitative methods. The program directly aligns with NHC’s mission in improving access to health and education by providing direct services to those that are experiencing symptoms of diabetes, obesity and need health education to improve their quality of life, prevent diabetes, reduce obesity and other chronic conditions in the underserved in South Chicago. The program specifically targets food insecure communities and those that are the most disparate in health outcomes. The program also focuses on connecting participants to health resources and other social drivers of health by improving access to identified services.
Major Duties and Responsibilities
Member Service Position major duties/ responsibilities-
Data Collection – assist with the completion of registration forms.
Data Entry– enter patient data from registration forms and survey’s in REDCap.
Farmacy Preparation – conduct follow-up reminder phone calls to all Food Farmacy participants, receive food from Greater Chicago Food Depository, set up food for distribution, hand out food items and set up and break down
Documentation - document and record all contact and follow-up activities in REDCap.
Virtual Evaluation – coordinate clinical screenings (blood pressure, weight, Physical activity) conduct ongoing review of quality of data, compile data and analyze data on a bi-weekly basis.
Equipment and Supplies – assist with the security of program supplies and equipment after each distribution by ensuring that equipment and supplies are properly placed in designated areas.
Program Planning – attend scheduled meetings with program staff for the planning of programs as needed.
Patient Education – provide educational material on the food provided food farmacy. Ex benefits off turnip greens providing vitamins and nutrition to the body

Knowledge of AmeriCorps/Health Corps member requirements

The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.
Review
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
