Job Description
- Provides individual contact with patients to identify and develop client directed
- Supports patients in navigating the clinic system and community resources.
- Engages patients expected to start ARV regimens in an assessment of readiness
effects and importance of adherence. Assesses patient needs upon onset of
medication.
- Provides individual and group educational skill building opportunities to foster
routines, communicates with providers on additional issues to increase
engagement in care and adherence to treatment;
- Enhances engagement in care and adherence by conducting patient reminder
- Meets with patients during clinical visits to introduce and discuss health promotion
HIV/AIDS education.
- Provides administrative assistance with clinical operations and medical records as
conduct HIV testing in clinic and community.
- Adheres to all confidentiality policies. It is a direct violation of company’s policy to
