Virtual Care Coordinator

Delivering great healthcare begins with you. That’s because our Patient Care Coordinator is an integral part of the vybe urgent care team! The Patient Care Coordinator is responsible for patient registration/triage within the telemedicine department and patient support in the call center as well as our online patient communication platforms and email.

You will lead the patient through registration and triage where you will virtually assist the patient and determine their healthcare needs by coordinating care with a Telemedicine clinician. It is essential for this role to be capable of coordinating and communicating effectively and multi-tasking during increased volumes or with complex patients. This position is on-site in our Bala Cynwyd office.

Key Responsibilities – Your Typical Day
  • Navigate in both athenaNet EMR and Doxy system to determine appropriate patient flow.
  • Coordinate patient care through video call.
  • Complete patient registration, insurance verification, and payment collection.
  • Screen and triage patient to include but not limited to chief complaint and symptoms, review of medications, allergies, past medical history, history of present illnesses.
  • Ensure thorough documentation of patient intake process.
  • Route patient to vybe urgent care telemedicine clinician.
  • Ongoing communication with clinicians to include notifying established contacts at vybe centers regarding patient transfer cases.
  • Administrative duties as needed including bucket completion, send patient notes and letters, patient call backs.
  • Assist other team members as needed.
Qualifications
  • High school diploma is required.
  • Experience on EMR, especially athenaNet is preferred
  • One year experience working as a medical assistant preferred.
  • Knowledge of telemedicine a plus. Must have intermediate skills in MS Word, MS Excel
  • Excellent organizational and interpersonal communication skills and ability to build and maintain effective working relationships across various constituencies.
  • Must convey a professional demeanor and be able to act calmly and effectively in a busy or stressful situation.
  • Bring a genuine high energy level and project a healthful image.
  • This position requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
