Virtual Office/Equipment Assignment

AWARE will issue the following equipment in establishing a Virtual Office: Lap Top Computer, Cell Phone, generic fine point stylus pen, and noted monthly stipend for printing, paper costs.

Equipment and vehicles are essential to accomplishing job duties, are expensive, and may be difficult to replace. When using company property, employees are expected to exercise care, perform required maintenance, and follow all operating instructions, safety standards, and guidelines. The improper, careless, negligent, destructive, or unsafe use or operation of equipment or vehicles, as well as excessive or avoidable traffic and parking violations, can result in disciplinary action.

Personal use of equipment and vehicles by staff or non-AWARE employees is prohibited unless specifically assigned and/or approved by the immediate supervisor. If an employee uses his/her personal vehicle for company purposes with proper authorization, the employee's personal auto liability insurance shall be considered the primary insurance policy, and AWARE’s auto liability insurance policy shall be secondary. Employees are discouraged from bringing personal property, including electronics or computers, to the workplace. AWARE is not responsible for loss or damage to an employee’s personal property.

Principal Duties & Responsibilities

  • Provide leadership, oversight, and technical assistance to SDMI Staff.
  • Receive and enter SDMI CM referrals into the electronic health record.
  • Evaluate SDMI service delivery, including conducting file reviews and coordinating quality assurance efforts.
  • Complete employee orientation, conducts and monitors training of SDMI CM’s.
  • Evaluate service coordination and recommend appropriate responses to ensure compliance with established goals, polices, and practices.
  • Attend community and state-wide meetings in an effort to promote case management services.
  • Research new training and development techniques and suggest enhancements to existing training and development programs.
  • Review Key Performance Indicator (KPI) reports in order to assure goals are met on a continuous basis and provide feedback to supervisor and SDMI CM’s.
  • Provide assistance in recruitment and interviewing SDMI CM’s.
  • Prepare reports, applications, and supporting documentation as requested by the supervisor within time lines established by that supervisor.
  • Work collaboratively with Service Administrator and Clinical Lead Program Supervisors to ensure compliance with SDMI CM program policy and objectives.
  • Conducts training and other developmental sessions in order to assure the growth, expansion, and enhancement of SDMI CM Services.
  • Participates in all orientation and training and passes testing required, or deemed necessary by the Organization.
  • May carry own case load and fill in for vacancies, and/or provide direct case management services for specified cases, as directed by the SDMI CM Service Director.
  • Performs other duties, as assigned.

Requirements

Required Skills, Abilities & Education:

  • Requires knowledge of case management techniques, staff training and development, leadership skills and abilities, and providing technical assistance using a variety of concepts, practices, and procedures.
  • Requires good oral and written communication skills and an ability to form relationships with a diverse group of people.
  • A BS/BA Degree in a Human Services, Business Administration, or related field, along with either two (2) years of direct AWARE SDMI CM employment experience or three (3) years in a similar service, such as adult case management, business, education and/or training.
