As EVP, Care Continuum Operations You Will

  • The EVP, under the direction of the CSDO will lead a senior team responsible for day-to-day operational execution, facilitating both operational and performance management and optimization. This includes working with CSDO to develop goals, objectives and priorities for Care Continuum Operations
  • The EVP’s primary responsibility is to ensure successful delivery and performance as well as collaboration among all departments in scope to deliver value to our client partners in the market.
  • The EVP will foster a culture that encourages engagement, teamwork and accountability, setting a high standard of leadership excellence within their area of responsibility.
  • The EVP will work closely among peers to ensure there is collaboration and integration across the solution operating model both internally and externally
  • The EVP will be accountable for core care continuum operational areas including: ambulatory care management, acute and post acute management, hospital throughput, in home services, palliative care, quality operations and clinical development and optimization.
  • The EVP will utilize existing and recommend new reports on performance insights that will drive value outcomes and performance in key performance indicators. The EVP will be responsible for presenting performance outcomes to key stakeholders both internal and external on care continuum operational outcomes, KPIs and impact
  • The EVP will work with direct reports to ensure the formulation and on going management of key initiative planning and implementation including ensuring there are operational plans and tactics with milestones and timelines to meet the needs of client partners as well as ensuring ongoing initiative and strategy development are in alignment with Navvis enterprise goals
  • The EVP will direct and evaluate the work of the subordinate management staff; establish and or evolve performance standards; educate, guide, counsel and develop management staff and address performance management concerns and technical issues.


A Day In The Life

Operational Leadership

  • Provide strong leadership and guidance to operational functions.
  • Monitor performance of operational areas as well as client partner KPIs working with operational teams on performance improvement strategies.
  • Identify best practice approaches and work with leaders to scale across client partnerships as appropriate.
  • Identify areas of opportunity to optimize care and performance across the care continuum
  • Ensure operational performance is meeting the contractual requirements of client partnerships and in scope performance standards are met


Budget Management

Manage Care Continuum Operations By Way Of

  • Budgeting across departments in scope
  • FTE planning, monitoring and re-assessing
  • Department expense management


Team Leadership

  • Continue to develop high-performing leadership team, providing coaching and mentorship to key managers.
  • Foster a collaborative and results-oriented culture that aligns with the company’s values and mission.
  • Promote professional development and talent retention.


Stakeholder Engagement

  • Cultivate and maintain strong relationships with key stakeholders.
  • Utilize a “First Team” approach in developing and maintaining peer relationships

  • A seasoned operations leader exemplifying collaborative and multidisciplinary leadership, serving as a role model who motivates, empowers and fosters creativity among the staff.
  • Extensive expertise in population health operations and organizational and operating models, bringing a wealth of knowledge and experience to the role.
  • Remaining well-informed about national best practices and have a track record of developing programs with success.
  • The ability to understand granular details of an operating model while keeping the strategic goals and bigger picture in mind to ensure operations is serving the needs of client partners
  • A passion for connecting the care continuum for the benefit of better patient care and experience.


Requirements

We are excited about you if you have these things:

  • Knowledge of EMRs, Data, Technology, Care Management, Analytics and other key population health functions
  • 10 years minimum experience in the healthcare industry
  • Operational leadership experience required
  • Knowledge of health system, health plan and physician enterprise operating models
  • Bachelor’s degree in business or relevant field required
  • Strong leadership skills at all organizational levels
  • Complex problem-solving skills in individual and group settings
  • Demonstrated success leading improvement initiatives
  • High expectations of others with a sense of fairness
  • Detail oriented with a compulsion to document
  • Operationally perceptive and politically savvy; sees what is ahead and navigates the pitfalls
  • Grit to endure process, grasp and understand details with a will to drive change
  • Ability to organize and distill complexity

  • Client facing with firsthand experience in operation implementation from the ground up
  • Deep knowledge of population health/value-based care and risked based performance tactics and strategies across the care continuum
  • Demonstrated knowledge and track record of leading population health management initiatives
  • Knowledge of Medicare Advantage, Medicare, Commercial, Medicaid, duals, full risk, capitation, shared savings, MSSP ACO and other lines of business and alternative payment models
  • Knowledge of operational aspects that make up a value-based care ecosystem such as care management, physician engagement, technology, analytics, networks, post-acute, hospital at home, etc.
  • Proficient in leading, directing, cultivating and assessing the performance of subordinate management staff and matrixed managers who report to other functional leaders.
  • Ability to analyze and evaluate results to determine whether activities effectively address and support care continuum operational objectives and produce the expected outcomes.
