Child and Family Team Facilitator

Overview

TheChild and Family Team (CFT) Facilitator's primary responsibility is to facilitate Child and Family Planning Meetings using wraparound principles that are family-centered, individualized, strengths-based, and outcome-focused. The program's goal is to facilitate Child and Family Team Meetings with a family-centered approach in order to assess, plan, intervene, monitor, and refine services over the course of the Departments involvement with the family. The CFT Facilitator role is to engage family members in the decision-making process and lead the team in discussion to work together to identify each family member's strengths and needs to develop a child, youth, and family-centered decision.

About Child And Family Team Partnership

Seneca's Child and Family Team Partnership provides facilitation for Child and Family Team Meetings (CFT) for Monterey County's Department of Child and Family Services (DCFS) and Juvenile Probation. Referrals are mde by Monterey County, and team meetings are scheduled and facilitated by Seneca. Our CFT Partnership also supports moving youth out of congregate care to focus on permanency. CFT meetings are held using a family-centered approach, and services are individualized, strengths-based, and outcome-focused.

Responsibilities

  • Facilitation of Child and Family Planning Meetings. This includes:
    • Preparing for the CFT meeting
    • Coordinating attendance
    • Create an agenda
    • Facilitating the meeting
    • Writing meeting notes and distributing them within two days
    • Cleaning up after the meeting
  • Collaborate with Social Worker/Probation Officer (SW/PO) and other team members to discuss pertinent information, identify CFT participants, prepare for the CFT, nad follow up afterwards regarding next steps.
  • Prepare a facilitator meeting summary documenting all decisions made in the CFT meeting in a timely manner, and maintain other documentation as indicated.
  • Determine and complete the Family Flex Fund Request form, gain signatures of and gather approval from the CFT participants, to meet an identified immediate need.

Qualifications

REQUIRED

  • Bachelor’s degree 2 years’ experience working with children/youth and families with complex and enduring needs in a mental health setting.
    • MHRS certification may be required , in accordance with county contracts for services.
  • At least 21 years of age.
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy.
  • Own vehicle and current automobile insurance.
  • Proof of up-to-date COVID-19 vaccination.

Preferred

  • Bilingual in English/Spanish preferred.
  • Ability to effectively work with a diverse population and workforce.
  • Strong organizational, time management, and writing skills.
  • Ability to work effectively as part of a multidisciplinary team.


Schedule

  • Full-time, hybrid role.
    • Services provided in-person; option to complete documentation remotely.
  • Monday - Friday, 9:00 AM - 5:30 PM.
    • One reoccurring evening shift per week, 11:00 AM - 7:30 PM
