Applications are invited for the post of Cancer Care Coordinator within the Lung cancer nursing team based at Wirral University Teaching Hospital NHS Trust. The post is part-time at 22.5 hours over 3 days per week to be worked as a job share with our existing staff member.

This is a pivotal role and is required to support the lung cancer team to coordinate the pathway for patients with cancer. You will be key to ensuring that cancer standards are met by proactively managing the patient pathway to ensure that patients receive their treatment in a timely and efficient way. This will involve liaising with clinical and administrative teams, both within the Trust and tertiary centres to coordinate appointments, investigations, and treatment. You will be communicating with patients and family members at a very challenging time, an empathetic, patient manner is required.

In addition, you will provide administrative support to the team and ensure all data requirements are collected and inputted onto the relevant database.

  • Provide regular supported conversations with the person living with cancer, assessing their holistic needs, and supporting them to self-manage by providing relevant information and advice, and signposting to other resources and services available to meet their identified needs
  • Participate in patient and public involvement activities to promote the service including awareness and promotional events outside of the centre.
  • Present / attend cancer conferences locally and regionally as required.
  • Organise the day to running of the service, organising meetings, bookings rooms, responding to email enquiries.
  • Ensure compliance with policies, procedures and clinical guidelines for self and others.
  • Work with other Co-ordinators across the region to adopt best practice
  • Use databases and computer packages to initiate and maintain records and generate statistics for analytical purposes
  • Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people
  • Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit
  • The post holder will demonstrate excellent organisational skills
