This exciting post is for a Band 6 Senior Occupational Therapist to work in the Memory Assessment diagnostic Service (MAS) providing cognitive and functional assessment that will aid in the diagnostic pathway for individuals over 65 years of age referred to the service with a suspected Dementia. You will be a valued part of a Multidisciplinary team and contributing to the diagnostic assessment.

MAS is a city/county wide service, based at Lings Bar Hospital and operating Monday to Friday. The candidate will need to be able to work autonomously and with flexibility across Nottinghamshire, utilising trust bases as required.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

As a senior member of the team you will be able to:

  • To work as part of the Multi Disciplinary Team (MDT) to triage and assess referrals to the service.
  • Contribute to Point of contact role.
  • To share information amongst the MDT and create appropriate and person-centred care plans.
  • To complete functional/cognitive assessments of individuals with a cognitive impairment to aid the diagnosis of Dementia.
  • To complete risk assessment and escalate/refer on when required.
  • To be part of a weekly/Monthly MDT meetings and feedback your assessments as part of the diagnostic pathway.
  • Liaise with Community teams to support referrals.
  • To complete accurate records/reports.
